The introduction says all the usage examples (e.g. of truncate_stream,
etc) in the section use [0,[1]] as input, but they actually use [1,[2]].
["a",["b"]] seems better than [1,[2]] as the string values won't be
confusable with the indexes in <path>.
I also changed the behavior of limit/2 with negative count to emit an error, like nth/2 does.
Also, I redefined nth/2 using skip/2 to minimize the impact of growth of builtin filters.

Mostly clarifications w.r.t. numbers.
The anomalous behavior of leading minus signs is documented, with an explicit notice that this will probably change.
Only examples defined in the "examples" field are generated, if they are
defined in "example", they are just ignored.
Also fix a bunch of incorrect tests.
When the string transformation to number failed, all following
transformation failed too.
This happend because status in decNumberFromString function is
updated just in error case. Reusing the DEC_CONTEXT that failed
before results into error even if the string is valid number.
Keep a cached copy of the man tests that we can use when no manpage
changes are made. This allows automated systems that might not have
easy access to a pipenv to build and run tests.
